Our specially trained healthcare providers will make sure you understand how your medications work, assist you in making lifestyle changes you can live with, explain what all the numbers mean, and help you learn how to live your life while optimally managing your diabetes.
Our goal is to help you and your family understand and control your diabetes with education and treatment.
What is Diabetes?
Diabetes is a metabolic disease in which the body’s inability to produce any or enough insulin causes elevated levels of glucose in the blood. Glucose is vital to your health because it's an important source of energy for the cells that make up your muscles and tissues. It's also your brain's main source of fuel.
If you have diabetes, no matter what type, it means you have too much glucose in your blood, although the causes may differ. Too much glucose can lead to serious health problems.
Your first appointment with our provider will include a physical exam, evaluation and diagnosis. You may refer yourself to the clinic or be referred by a physician.
To prepare for your appointment, please bring the following:
- A list of your medications
- If monitoring blood sugars, bring your meter or log book
- Lab results from your primary care physician
- Insurance information
We will communicate with your primary care provider about your plan of care, your progress and your concerns.
